Happy Independence Day!

And Welcome to my “New” Family Practice Location!

It has been quite an adventure getting to this point, and I am incredibly grateful for your patience and support. The wait has truly been worth it, and I am are thrilled to officially open the doors.

Practice Information

 

Hours of Operation:

  • Monday through Thursday: 8 AM - 5 PM

  • Fridays: 8 AM - 3 PM

Insurance:

We are pleased to accept most insurance plans. If you have any questions about your coverage or if we don't currently participate with your plan, please let us know, and we will do our best to assist you.

Appointments:

Appointments are booking up quickly, but we are committed to providing timely care. We have agreed to make same-day appointments available for any patient who calls before 9 AM with an acute need.

Medical Records Transfer

 

We are deeply grateful for the overwhelming interest in our practice and promise to do our best to keep you healthy. To ensure a smooth transition and comprehensive care, please send us your medical records using one of the following options:

Option 1:

Athena Portal (from previous Medical Facility)

Delivery Time: Average 1-3 days

  1. Schedule your appointment with our office by calling (248) 365-6064.

  2. We will need to collect new patient information to create a chart for you. Once this chart is created Athena will allow you to send records to us.

  3. Log in to your previous Medical Facility's Athena Portal. If you've forgotten your username or password, please contact your previous facility.

  4. Once logged in, you should arrive at the home screen.

  5. Click on "MyHealth" (located on the left side of the screen).

  6. Click on "Health Records" (the 4th tab on the top of the screen).

  7. Under "1. Choose the time frame select," click on "All Time."

  8. Under "2. Choose how you'd like to collect your data," click on "Direct Exchange."

  9. Enter the following Direct Email Address: Nathan.bloch.1@24172.direct.athenahealth.com

Option 2:

Obtain Medical Records through Mail (from previous Medical Facility)

 

Delivery Time: 2-4 weeks

  1. Call our office at (248) 365-6064 to get set up in our Electronic Records system and obtain an appointment slot.

  2. From this website, download our "Medical Records Release Form."

  3. Complete the form and fax it to our office at (248) 516-3145, attention Nathan Bloch, D.O.

  4. Complete another copy of the form and fax it directly to your previous Medical Facility’s Fax Number.

Welcoming my New Partner:

Dr. Hassan Kassem!

 

I am absolutely delighted to introduce and welcome Hassan Kassem, MD, as my new partner. Dr. Kassem is an amazing young man and a wonderful human being, and I couldn't be more excited to have him join me. Dr. Kassem and I share so much in common—the same healthcare interests, principles, and unwavering dedication to quality and patient care.

Dr. Kassem is a board-certified Family Medicine physician with extensive experience in hospital medicine, quality improvement, and primary care leadership. A proud metro Detroit native, he completed both his Bachelor's and Master's degrees at Wayne State University. He earned his medical degree from St. George’s University through the prestigious Keith B. Taylor Global Scholars Program and holds a Master of Business Administration with highest honors, specializing in multisector health management.

During his Family Medicine residency at Summa Health – Barberton Campus, Dr. Kassem distinguished himself, serving as Chief Resident and earning recognition as Resident of the Year. His leadership roles at Summa Health with US Acute Care Solutions include Hospital Medicine Site Director, Quality Director for Hospital Medicine, and Pre-Admission Testing (PAT) Director. While there, he spearheaded initiatives in quality metrics, care coordination, and performance improvement across large multisite hospital teams. He also contributed to various hospital committees, including the Peer Review Committee, Primary Care Task Force, Sepsis Hospital Committee, and Hospital Throughput Committee.

Dr. Kassem's clinical interests are comprehensive and include chronic disease management, joint injections, obesity management, and evidence-based care integration. Beyond his clinical practice, he is deeply committed to education and mentorship, serving as a Clinical Assistant Professor at NEOMED and precepting residents, medical students, and PA students. His dedication to teaching was recognized with the Teacher of the Year award at Summa Health in 2024. He is a proud member of the American Academy of Family Physicians (AAFP) and a Diplomate of the American Board of Family Medicine (DABFM).

Dr. Kassem approaches medicine with a strong commitment to compassionate, patient-centered, and value-based care. He believes in fostering long-term relationships with patients, empowering them with the knowledge and tools necessary to maintain wellness across every stage of life.

Outside of his professional life, Dr. Kassem is a devoted husband and father of two. He enjoys traveling, reading, cooking, and is an avid Detroit Lions fan.

APPROACH

Compassion and Commitment

Dr. Bloch believes in using a “tool box” to heal patients. He treats the whole patient and is willing to use a variety of tools, both traditional and holistic to find the right treatment for each patient. Dr. Bloch is skilled in Osteopathic Manipulation, joint injections, all types of skin treatments. Additionally, Dr. Bloch is an Assistant Clinical Professor of Family Medicine at Michigan State University, and is a leading trainer in Family Medicine in the Detroit area for medical students and residents from medical schools and programs all over the country. His reputation has created waiting lists for students trying to get into his training program. Known for his friendly bedside manner, Dr. Bloch has a strong patient following. He has a particular interest in orthopedics, opioid addiction, diabetic care, cardiac disease, and preventative care. Dr. Bloch is married to Rachel Bloch. They are the parents of four busy children. He enjoys movies, theatre and lake life in his spare time.

 

Since 2011, colleagues have recognized Dr. Bloch, as a Top Doc in the annual peer-to-peer survey to name the area’s top doctors. HOUR Detroit Magazine surveys more than 16,000 doctors in the greater Detroit metropolitan area who they recommend turning to for medical care. Dr. Bloch rose to the top as a provider of high-quality, compassionate primary care.

In 2019, Dr. Bloch was recognized as a Top Doc once more, receiving the most votes of all Family Medicine practitioners.
